How Long Off Work After Knee Surgery? Timeline by Job Type

Most people need 2–3 weeks off for desk work, 6–8 weeks for standing jobs, and 12+ weeks for heavy manual work after knee surgery. Your exact return-to-work date depends on whether you had a partial or total knee replacement, which knee was operated on, and whether you can return to light duties first.

The honest answer is that it depends far less on the operation and far more on what your job asks of your knee. As a rough rule: for a partial knee replacement plan a minimum of 2–3 weeks completely off with a phased return after that, and for a total knee replacement plan 4–6 weeks off plus a phased return. Below is a job-by-job guide Dr Oosthuizen uses with patients in Johannesburg when planning sick leave.

Desk and sedentary work (office, admin, call centre, IT): 2–3 weeks after a partial knee replacement and 3–4 weeks after a total. If you can work from home, elevate the leg on a stool and stand up every 30 minutes, many patients start part-time from home in week 2. Commuting — especially driving in traffic — is usually the limiting factor, not the work itself.

Work that mixes sitting and walking (teaching, retail floor supervision, healthcare admin, sales visits): 4–6 weeks after a partial and 6–8 weeks after a total. Expect swelling by the end of the day for the first month, so a half-day return before a full-day return is sensible.

Jobs on your feet all day (nursing, hospitality, hairdressing, security, shop floor, factory line): 6–8 weeks after a partial and 8–12 weeks after a total. Continuous standing on a fresh implant causes swelling and fatigue long before it causes any harm — the timeline is about comfort and safety, not damage.

Heavy manual and high-risk work (construction, mining, plumbing, farming, warehouse lifting, ladder or scaffold work, kneeling trades such as tiling): 12 weeks or more, and occasionally longer after a total knee replacement. Kneeling is the last thing to become comfortable and, for some patients after a total knee replacement, never becomes fully comfortable. If your job requires kneeling all day, raise it early so we can plan.

Professional drivers and long-distance commuting: driving returns at roughly 1–2 weeks for a left knee in an automatic vehicle and 3–4 weeks for a right knee, once reaction time and emergency-stop strength are normal. Long-haul or public transport driving usually needs 6–8 weeks and, for many operators, a formal fitness-to-drive clearance.

Practical paperwork: ask the rooms for a written medical certificate at your pre-operative visit so you can give HR notice well in advance. We can also issue a 'fit to work with restrictions' letter for a phased return — for example, half days, no ladders, no lifting over 10 kg, or no prolonged kneeling for a defined period. If you belong to a medical aid or have income protection cover, submit the certificate before surgery rather than after.

South African sick leave in practice: under the Basic Conditions of Employment Act you are entitled to six weeks' paid sick leave in every three-year cycle (based on the days you normally work in a six-week period). A knee replacement usually uses most or all of that cycle, so many patients combine sick leave with annual leave, unpaid leave or an income-protection claim. Employers may require a medical certificate for absences of more than two consecutive days — the rooms will issue one covering the full expected period.
